Social Worker (MH RRTP Case Manager)
The Social Worker (MH RRTP Case Manager) position within the West Texas VA Health Care System (WTVAHCS) is administratively within the Behavioral Health Service Line. The incumbent will provide face-to-face and/or telemental health services to Mental Health Residential Rehabilitation Program (MH RRTP) Veterans.
Serves as the case manager for assigned panel of Veterans.
Completes structured biopsychosocial assessments for Veterans in the Domiciliary for Veterans within identified timelines; typically within twenty-four to forty-eight hours of admission.
Completes risk assessments and engages in crisis intervention, as appropriate and necessary.
Provides education to patients, family members, and significant others regarding mental disorders and best practices approaches to treatment.
Provides education related to the Veteran's VA entitlements, eligibility, services and programs, as well as community resources for medical- and mental health needs.
Maintains regular contact with assigned Veterans, as clinically indicated (e.g., as long as ongoing care is requested).
Coordinates and develops the Veteran's treatment plan, incorporating input from the Veteran, and when appropriate, the family with the Veteran's consent.
Coordinates and develops the Veteran's discharge plan, incorporating input from the Veteran and Interdisciplinary team, as well as the family, when appropriate.
Collaborates with other service providers in assessing Veterans needs for VA services, programs and benefits; community services programs and benefits; meets with Veterans, individually to educate the Veteran and as appropriate Veteran's family, caregiver, and/or significant other about available services, as well as assisting with obtaining needed services and establishing appropriate referrals based on the Veteran's preference.
Ensures that referrals to other resources are completed and that the Veteran receives the appropriate level of care, either through the Department of Veterans (VA) or through community resources.
Acts as an advocate for the Veteran, and as appropriate the Veteran's family, caregiver and/or significant other; educates the Veteran, the Veteran's family, caregiver and/or significant other about methods of self advocacy and encourage the Veteran, the Veteran's family, caregiver and/or significant other to advocate on their own behalf to foster a sense of independence and empowerment.
Develops a resource file of VA and community-based social service programs.
Provides consultation to health care providers about Veterans, ensuring that care is coordinated, collaborative, and integrated within the WTVAHCS.
Monitors and documents implementation of the treatment plan, including tracking progress in the care delivered, the outcomes achieved, and the goals attained.

Pay:
Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
Parental Leave:
After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy:
After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
