Senior Psych Care provides collaborative behavioral health services to residents in long-term care facilities. Our multidisciplinary teams include Board-Certified Psychiatrists, Psychiatric Nurse Practitioners, Physicians Assistants, Licensed Psychologists, and Licensed Clinical Social Workers/Licensed Professional Counselors/Licensed Marriage and Family Therapists. We focus on continuity of care—not triage—ensuring our patients receive comprehensive, and compassionate services to achieve their best quality of life.
What You’ll Do- Conduct cognitive and emotional assessments to set/manage treatment plans.
- Manage psychotropic medication and gradual dose reductions.
- Collaborate with nursing home staff and SPC’s behavioral health team.
- Participate in treatment team meetings (in-person and virtual).
- Deliver care both in-person and via telehealth when appropriate.
- Maintain accurate, timely documentation in our EHR system.
- Collaborative team of specialists in geriatric mental health.
