Center Well Home Health is seeking a Home Health LVN to provide personalized, one-on-one nursing care in patients' homes under RN supervision. You will implement care plans, educate families, and document interventions to support recovery.
This role requires an active LPN/LVN license in the practicing state, CPR certification, and reliable transportation. A minimum of two years of clinical LVN experience is preferred, with home health experience a plus.
