Role
Home Health Licensed Vocational Nurse
Responsibilities- Deliver skilled in-home nursing care to seniors and adults with chronic conditions.
- Perform assessments, medication administration, wound care, and patient education.
- Collaborate with an interdisciplinary care team to develop and support care plans.
- Accurately document care activities and patient progress.

- Licensed Vocational Nurse certification.
