Job Overview

BAYADA Home Health Care is seeking a Registered Nurse (RN) for Home Health Admissions in Billerica, Massachusetts. This full‑time, ongoing staff position involves completing OASIS Start of Care (SOC) visits for adult and geriatric patients throughout the northeast region of Massachusetts.

• Complete OASIS SOC visits for adult and geriatric patients.
• Conduct head‑to‑toe assessments and identify subtle changes in patient condition.
• Utilize experience with acute and post‑acute care (e.g., CHF, COPD, wound care, IV therapy, post‑surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

• Work independently while knowing when to escalates concerns.
• Make real‑time clinical decisions without immediate on‑site support.
• Problem‑solve in unpredictable home environments.
• Manage a dynamic visit schedule and prioritize urgent visits.
• Use point‑of‑care documentation and technology efficiently.
• Maintain strong attention to detail while meeting productivity expectations.
• Adapt to varying home environments and demonstrate cultural sensitivity.

• Current nursing license in good standing in the state of Massachusetts.
• Graduated from an accredited and approved nursing program (transcript or diploma).
• At least one year of recent hands‑on medical experience within the past two years with the adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab units.
• Medicare‑certified home‑health experience and proficiency with OASIS documentation.

Competitive pay ranges with a weekly pay schedule. Base pay is $60‑70 per point, depending on qualifications. Flexible scheduling is available.
