Job DescriptionThe Memory Care Coordinator is the main point of contact for residents, families, and staff regarding resident care needs. The MCC must be an excellent communicator as they need to ensure all parties are appropriately informed. In this position, you will work closely with the community RN to ensure the accuracy of care for our treasured residents. The Memory Care Coordinator will also serve as the licensed administrator for memory care.
Responsibilities- Oversight of footsteps memory care program
- Maintaining accurate resident service plans, completing incident reports, hosting care conferences
- Monitoring alert charting and communication logs
- Works on as a caregiver or medication technician as needed
- Assuring our team of Caregivers and Med Techs are appropriately trained and supported
- Health Services employee scheduling
- Performance management and staff development
- Collaborating with life enrichment staff on planning activity programs for Memory Care
- Will be on call for periods of time
- Professional experience working with senior populations with dementia
- Experience in supervising staff within a healthcare or senior living environment
- Prior experience working in senior living, preferably involving medication administration
- Strong understanding of assisted living and long-term care principles
- Excellent communication skills to interact effectively with residents, families, and team members
- Ability to manage operations efficiently while maintaining a compassionate approach
- Memory Care regulations knowledge
- Excellent attendance and flexibility to work as needed
- Proficient PC skills, including e-mail, record keeping, routine database activity, word processing, spreadsheet, graphics, etc.
- Ability to maintain focus in a fast-paced environment
- Excellent interpersonal and team-building skills
- Sincere appreciation for elders and observable respect for caregivers, med techs, and the important work they do
- Ability to effectively present information and respond to questions from groups of managers, residents, employees, and the general public
- Ability to read, analyze, and interpret charting standards, physician orders, or administrative regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to prioritize, organize, and delegate assignments
- Ability to interpret a variety of instructions furnished in written, oral, electronic, or scheduled form
- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
This position is part of the greater health services team supporting senior residents in assisted living and memory care. The team includes the memory care coordinator, AL resident services coordinators, RN director of health services, and health care administrator.
