HomeHealth Overview
River Stone Home Health offers medical treatment for illness or injury to help patients recover, regain independence, and achieve self‑sufficiency. Services target chronic conditions such as heart disease, diabetes, pain management, and COPD, and include personal care. Care plans are developed in consultation with the patient’s health‑care provider and are delivered by a multidisciplinary team of nurses, therapists, social workers, and certified home‑health aides.
Job SummaryThe home‑health RN case manager delivers quality patient care through the nursing process—assessment, nursing diagnosis, planning, implementation, and evaluation—while coordinating case management, clinical documentation (OASIS), referrals, and customer satisfaction. The role supports the Clinical Manager and nursing staff, ensuring efficient, high‑quality service delivery.
Essential Functions and Responsibilities- Case Management (95%)
- Work independently and collaboratively to provide skilled nursing care and instruction, following clinical standards.
- Administer care, treatments, and medications per physician orders.
- Monitor and interpret symptoms, collaborating with other health‑care providers to develop and adjust care plans.
- Educate patients and families, teaching preventive care and medication review.
- Complete admissions and home visits, initiating priority‑setting in the plan of care.
- Consult and coordinate with the health‑care team throughout care implementation and evaluation.
- Reevaluate nursing needs regularly and revise care plans as needed.
- Address care, education, and discharge planning needs, documenting outcomes and potential improvements.
- Conduct supervisory visits with LPNs and CHHAs.
- Provide timely reports to the care team, including any changes in condition to physicians and families.
- Apply knowledge of universal precautions, safety measures, and Medicaid/Medicare regulations.
- Maintain electronic medical record entries in compliance with documentation standards.
- Utilize OASIS‑E and quality‑reimbursement measures when available.
- Engage in performance‑improvement activities and resolve patient or family complaints.
- Stay current with regulatory changes, HIPAA, and accreditation requirements.
- Participate in ongoing professional development and organizational training.
- Demonstrate interpersonal, cognitive, and technical competencies aligned with organizational values.
- Provide on‑call support outside office hours, carrying a program computer and telephone.
- Non‑Essential Functions (5%)
- Perform additional duties as assigned to support River Stone’s mission and goals.
- Minimum Qualifications
- Associate degree in Nursing.
- Relevant experience and training equivalent to required scope of knowledge, skills, and abilities.
- Preferred Qualifications
- Bachelor of Science in Nursing.
- Two years of nursing experience.
- Knowledge of nursing process and home‑care regulations.
- Current Registered Nurse license in Montana.
- Valid Montana Driver’s License and vehicle insurance (required for Hospice, Home Care, and Public Health).
- BLS and CPR certification.
- Computer literacy, Microsoft Office Suite.
- Calm, positive demeanor during difficult interactions.
