Position Overview
Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes acute, chronic, multiple, complex, catastrophic, or life‑threatening illnesses; combat stress; residuals of traumatic brain injury; community adjustment; addictions and other health problems.
Responsibilities- Coordinate care with multiple providers across all levels and sites of care.
- Address psychosocial, nursing, and medical needs of patients and their families/caregivers through participation in interdisciplinary patient care management practice.
- Evaluate care and outcomes to ensure timely and appropriate provision of services.
- Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
- Demonstrate performance and leadership that improves care for a group of patients.
- Provide peers with formal or informal constructive feedback for safe, quality nursing care.
- Foster a safe and supportive environment conducive to the professional development of health care professionals.
- Evaluate case management outcomes of evidence‑based decisions and practice changes for individuals, groups, and populations.
- Document case management outcomes at the program or service level.
- Practice as a Registered Nurse for the Montana VA Health Care System and provide direct support to the Community Nursing Home (CNH) program.
- Survey CNHs and provide follow‑up evaluations for VA patients placed in nursing homes.
- Review appropriateness for CNH placement.
- Represent Montana VA Health Care System with community contacts and maintain positive relationships with veterans, families, and the VA.
- Execute responsibilities that demonstrate leadership, experience, and creative approaches to managing complex client care within CNH.
- Collaborate with community agencies and VA staff to organize and coordinate clinical components of care, and request authorization of services and durable medical equipment in response to individual health care needs.
- Utilize all available avenues and resources to provide safe, efficient, cost‑effective use of health‑care resources and foster patient self‑managed care where possible.
- Possess intimate knowledge of federal regulations, VHA guidelines, medical review criteria, and utilization management processes.
- Assist with in‑services, orientation of new staff, and staff development.
- Oversee care coordination activities that meet the individualized needs of the veteran and national program requirements.
- One full and unrestricted Registered Nurse license from any U.S. State or territory.

Monday‑Friday, 8:00 AM–4:30 PM. Telework available on a limited ad‑hoc basis in compliance with the Return to Office Executive Order.
