Licensed Practical Nurse (LPN)

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our rehabilitation team. The ideal candidate will be passionate about providing exceptional patient care and committed to the recovery of individuals who have experienced severe injuries or illnesses.

The LPN will play a vital role in delivering ongoing care and specialized rehabilitation treatment to patients. This position involves collaborating with a multidisciplinary team to support the recovery journey of individuals dealing with various conditions, including but not limited to stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.

• Provide direct patient care under the supervision of registered nursing staff.
• Monitor patient conditions and report any changes to the healthcare team.
• Administer medications and treatments as prescribed.
• Assist in developing and implementing patient care plans tailored to individual needs.
• Maintain accurate and up-to-date patient records using electronic health records (EHR).
• Collaborate with physical therapists, occupational therapists, and other healthcare professionals to ensure comprehensive care.

• Current and valid LPN license.
• BLS certification is required.
• Experience in a rehabilitation or acute care setting preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

This full-time position guarantees 36 hours per week, with potential weekend hours. Shift times may vary based on department needs.

  Adults aged 16 years and older

• Time to Report: 6:45 AM
