American Traveler is hiring an experienced RN Case Manager for a hospital care management role requiring BSN and acute care experience.
Details- Hospital Care Management unit within an acute care hospital
- Day shift schedule, 5 days/week, 8:00a-4:30p (40 hrs/week)
- Uses Epic charting system
- Active RN license (Montana or Compact) required
- BSN required
- AHA-issued BLS certification required, must not expire prior to start
- Minimum 3 years of RN experience in an acute care setting required
- 1 year of experience in hospital care management required
- Knowledge of medical terminology required
- Proficiency with desktop software including PowerPoint and Excel required
- ACM or CCM certification preferred but not required
- 5 years of nursing experience preferred
- Coordinate care and services for patients identified as needing case management support
- Assess patients' medical, functional, and social conditions to develop individualized care plans
- Collaborate with physicians and multidisciplinary teams to plan and coordinate discharge
- Identify strategies to reduce length of stay and improve utilization of services
- Advocate for patient rights and monitor efficiency of care outcomes through data tracking
- Participate in rounds, case conferences, and multidisciplinary group meetings
- Provide education to physicians and staff regarding case management and discharge planning processes
- Former Intermountain permanent staff must be separated from the facility for at least one year before being eligible for this travel assignment
