Speech Pathologist
Provide service to patients who have speech and language problems. Participate in the development of speech therapy plans of care for patients. Make visits to patients for initial and ongoing assessments to identify their level of functioning and ability to communicate, including OASIS Assessments at appropriate time points. Communicate with the physician before the start of care and as needed. Implement the plans of care.
Provide education and instruction to team members, patients, and families. Document care and submit paperwork in an accurate and timely manner to update and maintain patient medical records.
