Role Overview:
The Registered Nurse in Home Health delivers and coordinates nursing care in patients' homes, following physician prescriptions and compliance standards. They collaborate with healthcare teams to develop, implement, and update patient care plans, conduct comprehensive assessments, and ensure quality clinical care. The RN supervises home health aides and LPNs, documents care accurately, and communicates effectively with patients, families, and providers. They participate in staff orientation, quality initiatives, and safety programs, demonstrating professionalism and adaptability in diverse home settings.
Qualifications & Skills- Valid RN licensure in the practicing state
- Reliable transportation and current driver's license
- Preferred: 1+ years clinical experience and CPR certification
- Develop and update patient care plans in collaboration with physicians and teams
- Provide high-quality care, supervise aides and LPNs, ensure regulatory compliance
- Accurately document patient interactions and coordinate care communication
- Participate in staff training, quality improvement, and safety initiatives
- Maintain professionalism, punctuality, ongoing learning, and adaptability in home environments
