This is a part time no benefits Social Worker position in an Article 31 OMH outpatient facility for one 8 hour shift a week. Located at 184 Court Street this is an active outpatient facility emphasizing a team approach to the best mental health care available.
Position SummarySupports psycho‐social needs of patients who are currently receiving or recently received hospital based care. The Social Worker strives to enhance the patient’s optimized level of health and/or recovery; provides interventions for populations ranging from infants to geriatrics; serves as a resource for the healthcare team, assists in identifying discharge planning needs; provides support / intervention and resources.
Education, License & Cert- MSW (Masters of Social Work) required.
- For staff providing care to patients in the state of New York, LMSW, LSW, LCSW is required.
- Minimum two years post graduate experience as a social worker in a similar type of health care setting is strongly preferred.
- Successful completion of a 6 month hospital based orientation process is required.
- Provides psychosocial evaluation to meet patient’s medically related social needs.
- Assessment of social and emotional factors related to the patient’s illness, care needs, response to treatment, and adjustment to care in the hospital.
- Assessment of patient’s medical and nursing requirements, home situation, financial resources, and available community resources in discharge planning decisions.
- Participates in unit interdisciplinary conferences to identify patient and family intervention needs.
- Identifies problems and develops plans based on evaluation of patient needs.
- Participates in an on‑call system for 24‑hour accessibility and accountability to meet patient needs.
- Receives training to provide care and services support resources to trauma patients.
- Responds to consults as required by policy.
- Facilitates discharge planning to meet patient needs for continued care (e.g., skilled nursing, rehabilitation, transfer, or other facilities).
- Identifies psycho‑social, environmental and financial problems affecting discharge planning.
- Collaborates with the healthcare team to establish discharge plans.
- Consults with Case Manager as necessary and requested.
- Keeps patient/family/referring facility informed of plan.
- Completes required documentation related to transfers and patient status; documents progress in the plan of care; ensures timely handoffs by end of each business day.
- Documentation includes all required regulatory components and reflects ongoing care progression.
- Assesses and facilitates financial needs; uses charity funds according to policies/procedures; assists patients/families with financial applications (PAMA, NYMAP, disability paperwork, etc.).
- Collaborates with Case Manager, Patient Business Office, etc. to meet financial needs.
- Provides crisis intervention and support for trauma and other psychosocial crises; assists with crisis intervention when on‑call.
- Provides interventions for identified high‑risk patients (e.g., abuse, neglect, substance abuse).
Performs other duties as assigned.
Pay range: $27.58–$43.70
