, a Modivcare Personal Care Service, is looking for a Per Diem RN Supervisor
- Home Care. You will have the opportunity to make a meaningful impact on the lives of our clients and foster the professional growth of our home health aides.

• Pay per Visit
  • Sup/Post Hospital: $60
  • Reassessment: $75
  • Initial Admission: $90 weekday / $125 weekend
• Visiting Client Territory: Binghamton and surrounding areas.
• Schedule: Visits planned with the required presence of the patient’s aide.
• Office Address: 65 Pennsylvania Ave Suite 220, Binghamton, NY 13903
• Mileage Reimbursement: Provided at the current federal rate

• Perform on-site supervisory and teaching visits to evaluate caregivers.
• Provide clinical orientation to caregivers on assigned home care cases.
• Participate in the training of new nurses and in‑services for field staff.
• Perform initial nursing assessments.
• Counsel patient/caregiver in meeting nursing and related needs.
• Assign and coordinate services provided.
• Assist physician in developing a plan of treatment/care and make necessary revisions based upon patient’s condition/needs.
• Prepare clinical progress notes ensuring any changes in the patient’s condition are reported.
• Monitor clinical records for complete and accurate documentation.

• Registered Nurse (RN)
• CPR Certified
• 1 year of Home Care experience required
• 1 year of supervisory or management experience required
• Valid NYS Driver’s License, insurance, registration, and reliable car
