Position Overview

United Health Services (UHS) is seeking a board‑certified Nurse Practitioner (NP) or Physician Assistant (PA) for a full‑time Monday‑Friday, Family Medicine Provider Float position. The role will have a home base at the UHS Primary Care Upper Front Street site and will include travel within a 30‑mile radius to assigned practices such as Upper Front St, Greene, and Clinton Street Medical Centers.

Compensation: $126,300 – $148,600 annually.

• Cover providers’ patient panels during disabilities, FMLA, or vacation time.
• Perform acute tele‑medicine visits from the home base practice.
• Assist in health promotion, disease prevention, health maintenance, and patient education for patients of all ages.
• Collaborate within a nationally recognized NCQA Certified Patient‑Centered Medical Home.
• Travel as required to assigned practices within a 30‑mile radius.

• Board‑certified Nurse Practitioner or Physician Assistant.
• Current New York State license as an RN and Family Nurse Practitioner (for NP) or a current New York State license to practice as a PA.
• Eligibility to attend UHS Advance Practice Residency Program for new graduates.
