Responsible for MDS coordination, Medicare determination, and ensuring compliance with all applicable policies and federal/state regulations. Facilitate QAPI and track nursing department analytics.

Monday - Friday 8:00 AM to 4:30 PM

Expected to assist clinically as needed.

• Graduate of an accredited School of Nursing
• Active New York State RN license (Required)

• Valid state certification as an MDS coordinator through a recognized organization, such as the AAPACN.
• RNAC preferred (Registered Nurse Assessment Coordinator)
• BLS and/or CPR certification preferred.
• Must be 18 years of age.
• Ability to read, write and speak the English language.

• Experience as a MDS Coordinator and long term care.
• This role also oversees our QAA/QAPI program so experience in Quality Assurance.
• A solid understanding of data collection, reporting, and analysis.
• Proficiency in MDS 3.0 assessments and procedures.
• A thorough understanding of medical-legal frameworks, such as the O  Act of 1987.
• The ability to draft and create thorough care plans for residents.
• Exceptional communication and analytical skills.
• An excellent work ethic and understanding of MDS procedures.
• Superb organizational and interpersonal skills.
• Patience and compassion.
