Supervisor

• Pay per Visit
  • Sup/Post Hospital:$60
  • Reassessment:$75
  • Initial Admission:$90 weekday/ $125weekend
• Visiting Client Territory:Binghamton, and surrounding areas.
• Schedule:Visits planned with the required presence of the patient's aide.
• Office Address:65 Pennsylvania Ave Suite 220, Binghamton, NY 13903
• Mileage Reimbursement:Provided at the current federal rate

• Performs on-site supervisory and teaching visits to evaluate caregivers
• Provides clinical orientation to caregivers on assigned home care cases.
• Participates in the training of new nurses and in-services for field staff.
• Performs initial nursing assessments
• Counsels patient/caregiver in meeting nursing and related needs.
• Assigns and coordinates services provided.
• Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
• Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
• Monitors clinical records for complete and accurate documentation.

• Registered Nurse (RN)
• CPR Certified
• 1 yearof
  
  Home Careexperiencerequired
• 1 year of supervisory or management experience required
• Valid NYS Driver’s License, insurance, registration, and reliable car
