Case Manager Assignment Duration

This is a 13-week contract position.

Day shift, 5x8 hour schedule (08:00 - 16:30), Monday to Friday. No weekend shifts required.

The Case Manager will be responsible for ensuring effective care coordination, conducting concurrent reviews, and determining medical necessity based on evidence-based guidelines. The role involves discharge planning and assessments of patient needs while maintaining compliance with HIPAA regulations.

• Care coordination and assessment of patient needs.
• Conducting concurrent reviews and continued stay reviews.
• Developing and implementing plans of care.
• Facilitating discharge planning.
• Utilizing Inter Qual Criteria and Milliman Guidelines.
• Ensuring compliance with CMS regulations.

• Minimum of 1 year of experience in case management.
• Charge experience preferred.
• No specific certifications required.
• Experience with Epic charting system preferred.
