The Southern Tier Independence Center is seeking a Rapid Transition Lead Housing Specialist to support individuals moving from homelessness to stable housing. You will conduct intakes, determine eligibility, assist enrollment, identify housing options, and coordinate with providers for ongoing support.
The Regional Lead will maintain a housing registry, track activities, report data, and serve as the regional contact for housing referrals and eligibility reviews.
