United Health Services is seeking a Social Worker to join the Psychiatric Services team at Binghamton General Hospital on a per diem basis. You will build therapeutic relationships, coordinate care, perform psychosocial assessments, and connect patients and families with resources to support recovery.
You will collaborate with physicians, nurses, Care Managers, and community agencies to develop individualized discharge plans and ensure smooth transitions with post-discharge resources.
