Full Time, Monday - Friday 8am - 5pm
Work Location:
Psychiatry Clinic, Substance Abuse Center / Hybrid
To coordinate and manage patient care within a clinical setting, serving behavioral health populations. To oversee the care plan for a caseload of patients and acts as a liaison among multidisciplinary team members. To ensure quality outcomes through collaboration with healthcare providers and support staff. To facilitate patient follow-up and continuity of care, and provides education and support to patients and families.
To operate under the general direction of a licensed medical provider and serves as a resource for internal and external partners involved in patient care.

Position RequirementsEDUCATION AND EXPERIENCE:
Bachelor's degree in Social Work, Community Health and Human Services or a related field required. Work experience may NOT substitute for education requirement.

and (11) maintain a customer focus and strives to satisfy the customer s perceived needs.
