Full-time / Monday - Friday - Salary: $50,000 - $60,000 Annually

We are seeking a compassionate and dedicated Social Worker to support the emotional, social, and practical needs of our residents and their families. This role plays an essential part in promoting resident well‑being, assisting with care planning, and ensuring a positive, supportive experience throughout each resident’s stay.

Key Responsibilities- Plan, develop, organize, implement, evaluate, supervise, and direct the social services programs and activities.
- Maintain an ongoing quality assurance program, develop and implement appropriate plans to correct deficiencies.
- Implement social service interventions that achieve treatment goals, address resident needs, link social supports, and physical care to enhance quality of life.
- Consult with and coordinate social services activities with administration, medical and nursing services, consultants, and other related departments, implementing changes as required.
- Ensure that necessary forms, reports, and progress notes are properly completed and contain informative descriptions of the services provided and the residents’ responses.
- Participate in community planning, development, and implementation of social care plans, resident assessments, and discharge planning to meet the needs of the facility, the residents, and their families.
- Coordinate the social services programs with residents, their families, staff members, and community agencies through interviews, consultations, and direct involvement.
- Provide information to residents and families concerning available support agencies and financial assistance programs.
- Maintain incident and accident reports related to behavior management and elopement.
- Maintain effective relationships and open communication with residents, families, staff, community, and outside healthcare resources.
- Participate in pre‑admission planning and placement of residents in the facility.
- Bachelor’s degree in Social Work or related field required.
- Experience in long‑term care, rehabilitation, or healthcare setting preferred.
- Knowledge of discharge planning processes and community resources.
- Strong documentation, organizational, and communication skills.
- Ability to manage sensitive situations with professionalism.
- Understanding of regulatory requirements in skilled nursing environments.
