The social worker will coordinate the interdisciplinary plan of care for patients from admission through discharge. They will also advocate for necessary resources and maintain accurate medical record documentation to ensure effective health plan implementation.

Create plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations and/or patient records. Advocate for resources and removal of barriers.
Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care. - What

Currently a Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
Bachelor's degree in Social Work required. Additional Preferences No additional preferences.

Candidates must hold a Bachelor's degree in Social Work and possess a current Licensed Bachelor Social Worker (LBSW) or Licensed Master Social Worker (LMSW) certification from the Alabama State Board of Social Work Examiners. The role requires strong organizational skills and the ability to integrate patient preferences into care plans.
