A value-based primary care organization focused on delivering concierge-level care to seniors without added costs is growing and expanding. Partnering with Medicare Advantage plans, the group is dedicated to improving outcomes through a patient-first, team-based approach and is seeking Geriatric Physicians to join their innovative care model near Montgomery, AL.
This Position Offers:
- Reduced patient panel size to 400 or fewer patients, allowing for more meaningful patient interactions
- Value-based, outcomes-driven care model with no fee-for-service pressure
- Collaborative, team-based environment including physicians, APPs, pharmacists, behavioral health, and care coordinators
- Strong administrative and clinical support, including coders and referral specialists
- Minimal call responsibilities
