Position Overview
Pay range: $35.52 - $52.07/ hour
Shifts: Days, Flexi/PRN
Job DescriptionTo coordinate an evidence-based comprehensive stroke program through program assessment, planning, organizing, multidisciplinary collaboration, and program evaluation for effectiveness and outcomes. To monitor Joint Commission Primary Stroke Center Certification requirements and ensure all program records and systems are compliant. To educate, coordinate, implement, and present community outreach and Stroke prevention programs and annual educational opportunities for program nurses and other care providers.
To plan, evaluate and grant continuing education credits for educational offerings, to develop standards for Stroke patient care in conjunction with the Stroke team, and to promote continuity of care from onset of stroke through rehabilitation.
