MDS Registered Nurse Job Summary

We are seeking a dedicated and compassionate MDS Registered Nurse to join our team at Pine Hill Rehabilitation and Wellness Center in Birmingham, AL. • Conduct daily assessments of patients' conditions and develop individualized care plans in collaboration with the interdisciplinary team
• Implement and evaluate the effectiveness of MDS assessments, care plans, and treatments to ensure optimal patient outcomes
• Provide direct patient care, including wound care, medication administration, and treatments as prescribed by physicians
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions and timely interventions
• Participate in quality improvement initiatives to enhance the overall quality of care and patient satisfaction

• Current registration with the Alabama State Board of Nursing (ASN) and a current RN license
• One year of experience in long-term care, rehabilitation, or acute care setting
• Strong communication, organizational, and time management skills

• Work in a fast-paced, dynamic environment with a focus on patient-centered care
• Collaborate with a multidisciplinary team of healthcare professionals
• Participate in continuing education and professional development opportunities to stay current with industry best practices
