Role SummaryThe purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare, keeping them healthy and minimizing hospital admissions. Nurse Practitioners work independently and collaborate with physicians and interdisciplinary teams within outpatient practices.
Core Responsibilities- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer annual wellness visits and health risk assessments with a holistic focus and precise documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health initiatives in collaboration with the Care Team (e.g., ensuring eligible females receive breast cancer screening every two years)
- Assist the care team with phone triage and outreach
- Educate patients on their health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed
- Perform other duties as assigned
- Master of Science in Nursing, Doctor of Nursing Practice, or graduate of an NP Certificate Program
- National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult‑Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active, non‑probationary state Nurse Practitioner license
- Active DEA license
- Valid U.S. work authorization
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Knowledge of HCC (Hierarchical Condition Categories) documentation, ICD‑10 coding, and Health Risk Assessments (HRAs)
- Team‑oriented and collaborative mindset
- Interest in an innovative, data‑driven population health model
- Bilingual proficiency in applicable languages
Typical pay range: $87,035 – $187,460 (base hourly rate or base annual salary) depending on experience, education, geography, and other factors.
