Registered Nurse – Transitional Care UnitA Med-Surg Role With Variety, Teamwork, and a 5:1 Ratio

Join our Transitional Care Unit, where nurses care for admitted medical-surgical patients who are awaiting placement in an inpatient bed. This unit offers the variety of the hospital setting with a defined scope of care and a manageable 5:1 nurse-to-patient ratio .

This is a great opportunity for nurses who enjoy medical-surgical nursing, patient education, care coordination, and working closely with a multidisciplinary team.

What You Can Expect5:1 nurse-to-patient ratio

Medical-surgical level patients

Admitted patients who have already completed their initial emergency evaluation

Collaboration with hospitalists, physicians, case management, pharmacy, respiratory therapy, and other support departments

Opportunities to strengthen assessment, time-management, and care-coordination skills

Clearly Defined Patient Acuity

This unit does not accept:

Stepdown patients

ICU patients

Patients requiring titratable drips

Patients needing a higher level of monitoring or intervention than a medical-surgical unit can safely provide

Patients requiring escalation of care will be transferred to the appropriate higher-acuity unit.

Primary Responsibilities

Complete ongoing nursing assessments

Administer medications and treatments

Monitor for changes in patient condition

Provide patient and family education

Coordinate care with the interdisciplinary team

Prepare patients for discharge or transfer to an inpatient medical-surgical bed

Support safe and efficient patient flow throughout the hospital

Job Summary

The RN Charge oversees the operations of a nursing unit during assigned shifts, ensuring effective and efficient delivery of high-quality patient care. This role supervises nursing staff, coordinates care with healthcare teams, and maintains compliance with healthcare regulations and hospital policies while fostering a positive work environment and promoting professional development.

Essential Functions
  • Supervises and supports nursing and support staff, delegating tasks and responsibilities to appropriate personnel.

  • Evaluates staff performance, providing feedback and coaching to maintain high standards of care.

  • Oversees the assessment, planning, implementation, and evaluation of patient care, ensuring patient care plans are followed and adjusted as needed.

  • Collaborates with healthcare professionals to coordinate patient care and resolve issues related to patient flow or care delivery.

  • Ensures effective communication among physicians, patients, families, and hospital departments, including managing shift handoffs and unit huddles.

  • Monitors patient flow, manages bed assignments, and optimizes resource utilization within the unit.

  • Conducts employee and patient rounding to evaluate care delivery and satisfaction.

  • Monitors adherence to safety standards, infection control protocols, and hospital policies, addressing non-compliance promptly.

  • Participates in quality improvement initiatives and audits to enhance patient outcomes and ensure compliance with healthcare regulations.
