Hospice RN Care Manager: Lead Patient Care

Gentiva is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.

The RN will assess and coordinate patient and family needs, serve on the IDG, and support documentation, compliance, and care planning in a fast-paced field setting.
