- Assist in pre‑admission and admission processes with customers, including providing tours and signing paperwork.
- Interview clients and families to determine financial status and need for Medicaid assistance.
- Assist families in applying for clients’ Medicaid (ICP) approval and attend annual Medicaid eligibility reviews when clients cannot attend.
- Maintain tracking and reporting of Medicaid application statuses alongside the facility bookkeeper.
- Support the Social Services Director with interviewing residents and families to obtain relevant past and present social histories.
- Document progress and update plans of care at interdisciplinary conferences.
- Provide one‑on‑one counseling or group discussions to help residents and families adjust to admission.
- Assist the Social Services Director with planning post‑discharge services and follow‑up care.
- Maintain effective communication with residents, families, health‑care professionals, and facility staff.
- Assist residents and families in any situation as needed.
- Handle protected health information in financial and medical records as required.
- Minimum high school diploma or equivalent; bachelor’s degree in social work or human services preferred.
- One year of supervised social work experience in a health‑care setting, working directly with individuals.
- Experience or knowledge of the Institutional Medicaid Program (ICP) and its application process preferred.
- Ability to work with a variety of people in community and health‑care settings.
- Ability to make decisions concerning procedures based on evaluations and interpretations of clients’ case histories.
