RN- MDS Coordinator

• Complete the assigned nursing portions of the MDS and sign forms to confirm accuracy and completion.
• Review all sections of the RAI/MDS for validity and perform final quality checks before submission.
• Maintain resident dignity, privacy, confidentiality, and promote a safe, pleasant living environment.
• Communicate with physicians, residents, families, and interdisciplinary team regarding resident conditions.
• Observe, assess, and report changes in resident status according to facility standards.
• Educate co-workers and participate in support related to the RAI/MDS process.
• Utilize PCC EMR accurately for documentation and assessments.
• Ensure compliance with facility policies, regulatory standards, and care plan requirements.

• Graduate of an accredited School of Nursing.
• RN license for state of practice required.
• Knowledge and experience with the RAI/MDS process.
• Must have LTC MDS Coordinator experience.
• Must obtain RAC-CT (AANAC) certification within 6 months if not already certified.
• Minimum 2 years relevant experience.
