Job Summary

Sanford Health is seeking a compassionate and skilled Inpatient Case Manager to support patients through their hospital stay, coordinate care across disciplines, and ensure safe, seamless transitions from inpatient care to the next level of services. This role offers predictable schedules with limited weekends or holidays (as applicable).

Responsibilities
  • Provide expertise and leadership to manage patient care resources across the continuum for assigned populations.
  • Collaborate with patients, families, practitioners, and inter‑professional teams to coordinate care, transitional care planning, and discharge needs.
  • Enhance clinical outcomes and patient satisfaction while controlling cost of care.
  • Promote patient wellness through evidence‑based practice, efficient utilization of health services, and appropriate level of care during hospitalization and post‑discharge.
  • Provide discharge‑planning education and refer to inter‑professional teams as needed.
  • Initiate links with post‑acute external care resources and anticipate patient educational needs.
  • Act as advocate for patients across the continuum of care to achieve desired outcomes.
  • Utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, evaluation) to provide individualized nursing care.
  • Collaborate with physicians and other colleagues to plan, implement, and evaluate care.
  • Deliver care throughout all phases of preventative care, health maintenance, diagnosis, treatment, and follow‑up.
  • Plan and coordinate care, assessment, education, triage, and other nursing interventions within scope and standards of nursing practice.
Qualifications
  • Bachelor's degree in Nursing (preferred in Bemidji, WI; Michigan; Rapid City, SD; required in Fargo and Sioux Falls, SD).
  • Minimum of two years of clinical experience preferred.
  • Current, unencumbered RN license with the State Board of Nursing of the state of practice; multistate licensure acceptable for NLC states.
  • Must complete Basic Life Support (BLS) within 60 days of hire.
  • Case Management certification (CCM) or specialty certification is encouraged if eligible.
  • Approval from the Nursing Executive may be required for waivers of Bachelor’s degree requirements on a case‑by‑case basis.
