LMSW Social Worker
Start Date:
8/24/2026 Pay Rate: $1600.00 - $2400.00
Assists with patient intake and scheduling support, insurance and financial assistance coordination, transportation arrangements, community resource referrals, discharge and transition planning, patient advocacy, documentation, and communication between patients, families, and the dialysis care team. Float to sister facilities.
MinimumRequired Qualifications –
- ND LMSW or LCSW
- BLS
- 1+ year experience in specialty
13 weeks
SHIFT / HOURS PER WEEK8a-5p, depending on travel or tele visit options for sister locations
