• Must have an active, unencumbered LPN/LVN license in the state where the clinician will practice
• Compact licenses must be transferred to the state of residence within 90 days
• Current CPR certification (hands‑on component)
• TB skin test current within the last 12 months
• Six months of hands‑on nursing experience preferred but not required
• Must have reliable transportation

• Provide one‑to‑one patient care in a home environment
• Execute physicians’ prescribed plans of care and document care in the system of record
• Administer prescribed medication, treatments, and therapies
• Conduct patient assessments and coordinate care
• Promote health, teach, and train family members
