• Perform comprehensive PT evaluations
• Develop and adjust individualized treatment plans
• Deliver skilled therapy in the home setting
• Complete accurate, timely documentation
• Collaborate with nurses, aides, and physicians
• Educate patients and caregivers on safety, mobility, and exercises

• Active Virginia PT license
• Minimum 1 year PT experience (home health preferred)
• Strong clinical judgment and communication skills
• Ability to work independently
• Valid driver’s license & reliable transportation
