Description

The Memory Care Manager plays a vital role in ensuring the delivery of exceptional care to residents within the memory care unit. This position serves as the primary point of responsibility for all residents in memory care, overseeing daily operations, staff performance, and coordination of care services. The Supervisor works closely with healthcare providers, families, and interdisciplinary teams to support residents’ physical, emotional, and cognitive well-being.

In addition, this role partners with the Life Enrichment team to ensure that residents experience a meaningful daily flow of engagement through the Twenty/20 Signature Memory Care programs. The Manager helps guide staff in integrating purposeful engagement into all aspects of care, fostering a culture where connection and quality of life are central to the resident experience. The Memory Care Manager may be a licensed nurse qualified to act as a main support in the absence of the Health and Wellness Director.

Administrative support for clinical leadership, and on-call or rotating support as a main clinical POC or communication liaison during staffing constraints, holidays or other coverage support needed is required.

Requirements
  • Current Medication Technician certification training as per state specific requirements.
  • Prior experience in memory care or geriatric nursing is preferred .
  • CPR and First Aid certification (must be obtained within 60 days of employment).
  • Documented TB screening and acceptable criminal background check, as per state requirements.
  • Strong leadership, organizational skills , and time management skills.
  • Effective verbal and written communication abilities.
  • Compassionate, professional demeanor with a commitment to resident-centered care.
  • Ability to work independently and collaboratively across departments.
  • Willingness to obtain Dementia Connection Specialist (DCS) certification within the first year of employment.
Summary RESPONSIBILITIES
  • Provide direct and indirect care to residents, ensuring safety, dignity, and quality of life .
  • Serve as the designated person responsible for all residents in memory care, maintaining a consistent presence and availability to residents, families, and staff.
  • Assist the Department Head and other leaders as needed.
  • Support department director in supervising and training caregiving staff / med-techs, and other team members.
  • Collaborate with physicians, home health agencies, hospice providers, and families to ensure residents’ care needs are met.
  • Perform duties as needed in caregiving, medication administration, activities coordination, and housekeeping.
  • Support staff in implementing The Lavender Way and other Twenty/20 Signature Memory Care programs, ensuring engagement opportunities are integrated throughout the daily routine.
  • Work in close partnership with the Head of Life Enrichment to promote a culture of engagement, coaching staff to view every interaction as an opportunity for meaningful connection.
  • Encourage and model company core values and best practices , including in dementia care through a willingness to become Dementia Connection Specialist (DCS) certified.
  • Any other tasks or duties assigned by your manager, and support for other departments as needed.
CLINICAL & ADMINISTRATIVE DUTIES
  • Order, verify, and manage resident medications, including monitoring expiration dates and proper disposal.
  • Maintain accurate and up-to-date resident charts, care plans, and physician orders.
  • Conduct daily audits of the electronic medication administration record ( eMAR ) system to ensure documentation accuracy and compliance.
  • Monitor and report changes in residents’ conditions to physicians and care teams.
  • Ensure completion of medication administration, treatments, and routine rounds.
  • Delegate tasks appropriately to med-techs and caregivers.
  • Act as a mandated reporter for adult abuse and report any safety concerns promptly.
  • Communicate professionally with physicians and other healthcare providers via fax and other correspondence.
