As a Home Health Registered Nurse, you’ll provide skilled, individualized care in the comfort of your patients’ homes, helping them recover from illness, manage chronic conditions, prevent hospitalizations, and maintain their independence.

• Perform comprehensive patient assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, medication management, wound care, and patient education
• Monitor patient progress and communication changes to physicians and the interdisciplinary care team
• Coordinate care with therapists, social workers, home health aides, and other healthcare professionals
• Complete timely and accurate clinical documentation
• Educate patients and caregivers to promote health, safety, and independence

• Active Virginia Registered Nurse (RN) License Required
• Valid Driver’s License and Reliable Transportation
• Strong Clinical Assessment and Critical Thinking Skills
• Excellent Communication and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS experience is a plus
