Do you have a passion for engaging with people and helping them on their journey to better health? Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engage with people in the comfort of their own homes, and bridge gaps in care.
The visit, which lasts about 45 minutes, includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests. You will use an iPad and an intuitive clinical workflow for each evaluation and are not required to prescribe medicine, order lab tests or alter people’s current treatment regimen.
Responsibilities- Provide in‑home health evaluations (45‑minute visits) and support patients with medication and medical history review, physical evaluation, and diagnostic tests.
- Use an iPad and intuitive clinical workflow to conduct assessments.
- Bring your heart into every visit as a national network clinician dedicated to improving health outcomes.
- Provide connections to the right care based on unique patient needs.
- Travel locally and within licensed states; flexible to adapt to virtual visits as needed.
- Deliver the specified Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening, etc.).
- Work closely with Signify Health clinicians and fulfill role responsibilities.
As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to confidently and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants.
Qualifications- Active, unrestricted license(s) in the coverage area(s).
- Board certification (required).
- Multi‑state licenses (preferred).
- 2080 hours completed for all Advanced Practice Clinicians.
- Capability to serve primary locations:
Minneapolis and surrounding area or other areas of Minnesota. - MD/DO, NP or PA with appropriate experience.
Anticipated weekly hours: 40; full‑time.
Pay range: $87,035.00 – $. The actual base salary will depend on experience, education, geography and other factors.
