ABOUT THE JOB

Olivia Hospital & Clinic is looking for a Registered Nurse, Ambulatory Clinic. This is a full-time position working 72 hours per pay period. This position utilizes a systematic approach to nursing practice incorporating assessment, planning, nursing diagnosis, implementation and evaluation in accordance with the Nurse Practice Act, scope of practice, along with recognized professional standards and Health Partner and employer specific protocols and policy.

This position supports multiple areas within a clinic setting with strong interpersonal skills and flexibility while maintaining patient confidentiality and protecting patient rights. Resolves concerns or complaints regarding patient care or service.

Responsibilities:
  • Effectively assess care needs of patients and caregivers, prioritize patient needs utilizing the nursing process; determine nursing diagnosis focused on care and identify patient outcomes; develop patient care plan; implement and continuously evaluate the care plan; ensure patients are discharged with resolved educational needs
  • Collect and prioritize data in a systematic and ongoing process that involves the patient, caregiver, other health care providers and environment as appropriate
  • Use appropriate evidence-based assessment techniques and instruments in data collection
  • Analyze and interpret assessment data to determine nursing diagnoses
  • Formulate and revise nursing diagnoses that reflect the current patient status and communicate this with the patient, caregiver and other colleagues within the health care team
  • Develop plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
  • Establish a plan that provides for continuity of care
  • Develop an individualized education plan for patients and caregivers based on identified learning needs
  • Use evidence-based research data blended with caring and compassion, to develop the plan of care and interventions
  • Coordinate, deliver, and delegate nursing care that is safe, ethical, and culturally competent based on patient assessment/standards of care in a timely manner
  • Utilize systematic and ongoing assessment data to revise diagnoses, outcomes and the plan of care
  • Involve the patient, caregiver, and health care team members in the evaluation process when appropriate
  • Utilize evidence-based interventions and treatments specific to the diagnoses as appropriate
  • Initiate and implement plan of care using an outcomes-based approach ensuring quality care and age-related growth and development principles
  • Provide comprehensive, individualized healthcare teaching, using the teach back method, to patients and caregivers
  • Actively participate in creating and implementing improvements at the point of care to achieve clinical quality, patient experience, nursing engagement, efficiency, and effective outcomes
  • Evaluate the patient’s progress toward attainment of the outcomes and collaborate with members of the care team when outcomes are not as expected
  • Evaluate and document the patient’s/caregivers’ understanding of and response to the plan of care
  • Document patient care and education accurately, timely, and completely in the EHR (electronic health record)
  • Reassess, revise, and document the plan of care based on the patient’s response to treatment and nursing interventions on an ongoing basis
  • Coordinate and communicate effectively with patient/caregiver, clinicians, and the interdisciplinary team in a timely fashion
  • Delegate tasks consistent with the State Nurse Practice Act based on the knowledge, skills and abilities of the designated caregiver
  • Direct, supervise, and evaluate the work of unlicensed assistive personnel in providing delegated nursing services
  • Collaborate/coordinate with other healthcare team members, patient and caregivers to determine necessary resources (i.e. social services, home care, type of equipment, information) to support continuity of care
ABOUT YOU
  • Graduated from an accredited school of nursing.
  • Associate's Degree or diploma required.
  • 1 year of prior experience in nursing preferred.
  • Maintain a current Minnesota registered nurse's licensure. WI licensure where applicable.
