Job SummaryWe are currently hiring a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site care to residents in long‑term care settings in Utica, NY. This is a full‑time position ideal for a provider who values meaningful patient relationships and a collaborative care environment. The provider will manage a daily census of 25–30 residents across the facility, 5 days per week.

Compensation- Full‑Time salary range: $125,000 – $200,000 per year
- Manage and provide primary care to a daily census of 25–30 residents
- Provide primary care services to residents in skilled nursing and long‑term care facilities
- Perform physical exams and complete medical history assessments
- Diagnose and treat chronic and acute health conditions
- Order and interpret diagnostic tests and laboratory results
- Develop and adjust individualized care plans
- Prescribe medications and monitor treatment effectiveness
- Collaborate with facility staff and interdisciplinary care teams
- Educate patients and families about treatment options and care goals
- Ensure a safe, respectful, and supportive environment for all residents
- Active and unrestricted Nurse Practitioner or Physician Assistant license
- State-specific license in good standing in New York
- Minimum 6 months of experience in long‑term care or a skilled nursing facility
- Strong communication, clinical, and critical thinking skills
- Passion for elder care and person-centered healthcare
- Ability to lift a minimum of 20 pounds and remain on feet for up to 6–8 hours per day
