We are currently hiring a Primary Care Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site care to residents in long‑term care settings in Utica, NY. This is a full‑time position ideal for a provider who values meaningful patient relationships and a collaborative care environment. The provider will manage a daily census of 25–30 residents across the facility, 5 days per week.

• Full‑Time salary range: $125,000 – $200,000 per year

• Manage and provide primary care to a daily census of 25–30 residents
• Provide primary care services to residents in skilled nursing and long‑term care facilities
• Perform physical exams and complete medical history assessments
• Diagnose and treat chronic and acute health conditions
• Order and interpret diagnostic tests and laboratory results
• Develop and adjust individualized care plans
• Prescribe medications and monitor treatment effectiveness
• Collaborate with facility staff and interdisciplinary care teams
• Educate patients and families about treatment options and care goals
• Ensure a safe, respectful, and supportive environment for all residents

• Active and unrestricted Nurse Practitioner or Physician Assistant license
• State-specific license in good standing in New York
• Minimum 6 months of experience in long‑term care or a skilled nursing facility
• Strong communication, clinical, and critical thinking skills
• Passion for elder care and person-centered healthcare
• Ability to lift a minimum of 20 pounds and remain on feet for up to 6–8 hours per day
