** Registered Nurse or Licensed Practical Nurse Home Health | Position Details
** The RN or LPN Case Manager provides skilled nursing care, patient education, and care coordination in collaboration with the healthcare team to support optimal patient outcomes.
* Assess patient and family needs and develop individualized care plans
* Deliver skilled nursing care and health teaching
* Monitor patient condition and report significant changes
* Maintain accurate documentation of visits and care provided
* Coordinate care with physicians and interdisciplinary team
* Supervise Home Health Aides and support team collaboration
* Participate in case coordination, quality improvement, and staff support
* Assist with staff training, onboarding, and performance evaluation
* Perform additional duties and leadership responsibilities as assigned
** What You Offer
*** Graduate of an accredited nursing program
* Current RN or LPN license in Minnesota
* Valid driver’s license and auto insurance; ability to travel in all weather conditions
* Strong clinical, communication, and organizational skills
* Ability to work independently and maintain professionalism and confidentiality
* Communicate effectively in English, verbally and in writing.
* Pass a background study through the Minnesota Department of Human Services.
* Patient-focused care and strong clinical judgment
** Work Schedule
*** Day Shift - 8am-4:30pm
* Casual or Roster Status
** Compensation
* ** Registered Nurse: $41.82-$57.64 DOE
* Licensed Practical Nurse: $26.99-$37.21 DOE
