PositionSummary
We are seeking a Hospice Visit Nurse (RN) who thrives in the field, values patient relationships, and is excited to grow with a mission‑driven organization. This role will begin as part‑time, ideal for a nurse seeking flexibility, and transition to full‑time as we onboard additional clients. This is a unique opportunity to be part of something new, meaningful, and built to last.
Key Responsibilities- Travel to patients’ homes, assisted living, or memory care settings to deliver routine and PRN visits
- Provide symptom management, pain control, wound care, and clinical assessments
- Support patients and families through active listening, education, and compassionate communication
- Collaborate with the interdisciplinary team, including MDs, social workers, chaplains, and intake staff
- Maintain accurate and timely documentation in the EMR system
- Active RN license in the State of Minnesota
- Hospice experience highly preferred (will also consider strong backgrounds in oncology, palliative, or med‑surg with end‑of‑life exposure)
- Ability to work independently in the field and adapt to a flexible, growing caseload
- Excellent communication skills and emotional intelligence
- Reliable transportation and a valid driver’s license
