Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments across communities. You will review medical histories, medications, and social determinants to close care gaps with a single visit.
Responsibilities include performing adult/geriatric assessments, ordering testing, and collaborating with physicians and care teams. Travel up to 80-90% with overnight stays may be required to serve members in their homes and SNFs.
