St. Croix Hospice in Blaine, MN is seeking a compassionate RN Case Manager to lead the patient’s hospice care team and develop the Plan of Care with the Hospice Medical Director, patient, family, and facility staff.
You will provide day-to-day patient care, administer medications, maintain records, coordinate with physicians and interdisciplinary teams, and support families to enhance quality of life in a home-like setting. On-call availability may be required.
