In-Home Health - Nurse Practitioner or Physician Assistant; Part Time

Role

Part-Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient outcomes.

• Conduct 45-minute in-home health assessments, including medical history review, physical evaluation, and optional diagnostic tests.
• Utilize an iPad and clinical workflow; no prescribing, ordering labs, or changing treatments required.
• Build trusted relationships with patients and connect them to appropriate care.
• Perform virtual visits as needed and travel within licensed areas.
• Deliver Diagnostic & Preventive Services like spirometry, bone density screening, etc.

• Active, unrestricted licenses in designated areas.
• Board certification required; multi-state licenses preferred.
• Ability to work in diverse home environments with pets, smoke, or allergens.

Engage with patients in their homes to promote health, provide personalized evaluations, and support care bridging.
