Lutheran Family Services is forming a new team in Fremont, Nebraska. The Assertive Community Treatment (ACT) team utilizes a multidisciplinary, team-based service delivery model that offers continuous, community-based support to individuals with serious mental illness, particularly those with concurrent substance use issues, frequent hospitalizations, risk of homelessness, involvement with the criminal justice system, and psychiatric crises.

ACT is where care meets the community. Team members engage with individuals in various settings, including the office, homes, streets, shelters, and community spaces. This approach fosters genuine relationships while providing accessible, consistent, compassionate, and deep human support.

Position Summary:

• A Case Manager provides direct services to the residents of Dana Village, including those participating in the Root + Rise program that serves former foster youth residing in Independent Living. The overarching goal is to provide case management services to individuals and their dependents to create both short and long-term goals leading to self-sufficiency and self-reliance.

Job Duties:

• Provide quality and timely client-centered services to all program participants, including but not limited to education, finance, health, legal, and social services
• Maintain regular contact to ensure client needs are met and progressing towards self-sufficiency.
• Comply with funder, program, and organization requirements.
• Work closely with health, education, and social service providers in the area to provide holistic case management services.
• Complete individualized assessments of clients, addressing both short-term and long-term needs.
• Document and track client’s progress, case notes, and required data in appropriate systems in a timely manner.
• Attend and participate in staff meetings for training, team building, and program planning as required.
• Participate in regular supervision meetings discuss case, strategy, monitor client progress, etc.
• Utilize resources for addressing difficult client situations.
• Work closely with employment and education staff to ensure a client-centered holistic array of care.
• Demonstrate respect for all people, families, and communities.
• Ensure adherence to service provision standards and program outcomes.
• Other duties as assigned.
