Licensed Practical Nurse (LPN)

Description
  • Schedules and coordinates patient/doctor appointments.
  • Assists doctors and Nurse Practitioners with examinations, treatments, special tests, and procedures.
  • Reviews patient charts, checks patients in, obtains and records patients' weight, blood pressure, urine samples, blood samples, and medical history, and assesses reasons for patient visits.
  • Records and reports observed symptoms, reactions, treatments, and changes in the patients' conditions.
  • Perform a variety of pre-examination work on patients; observe patient condition; record patients' medical histories and immunizations; administer blood pressure, hearing tests, vision tests, urinalysis, and other pre-examinations as necessary; measure patients' height and weight; prepare patient charts and record pertinent information.
  • Transports patients to and from vehicles via wheelchairs.
  • Administers medications; applies sterile dressings.
  • Maintains and stocks inventory of office and/or clinical supplies, as appropriate to the position.
  • Educates patients regarding health-related issues.
  • Co‑ordinates labeling, delivery, and receipt of laboratory specimens and results; provides information and co‑ordinates follow‑up visits regarding laboratory results.
  • Maintain stock and order laboratory supplies.
  • Transports labs to hospital or delegates this task.
  • Conducts weekly chart audits, minimum of 10.
  • Documents QI indicators in E-Clinical.
  • Co‑ordinates referrals and transportation to these appointments if needed.
  • Arranges appointments for radiology test and procedures.
  • Monitors and maintains GRITS inventory. Performs monthly inventory count and submits to VFC.
  • Supervise and delegates duties to MA and CNA.
  • In the event of MA or CNA absence, delegates the absentees routine task to others.
  • Performs telephone triage and assesses patient problem and acts according to written protocols. Answers nurse calls, responds to patient inquiries regarding prescriptions, lab results.
  • Monitors and maintains AED, crash cart and O2. Documents in crash cart log.
  • Screens provider phone calls from hospital, nursing home and other affiliates if provider is in room with a patient.
  • Processes pharmacy prior authorizations.
  • Practices safety, environmental, and/or infection control methods.
  • Performs miscellaneous job-related duties as assigned.
Position Requirements
  • Vocational / technical training in Nursing; no previous work experience required.
  • Successful candidate must submit to post‑offer, pre‑employment physical examination/medical history check.
  • CPR Certified.
  • Knowledge of patient care charts and patient histories.
  • Knowledge of clinical and/or surgical operations and documentation.
  • Skill in performing clinical and medical procedures.
  • Knowledge of patient evaluation and triage procedures.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of supplies, equipment, and/or services ordering and inventory control.
  • Ability to safely lift, and physically manipulate patients.
  • Ability to maintain calendars and schedule appointments.
  • Knowledge of related accreditation and certification requirements.
  • Knowledge of appropriate procedures and standards for the administration of medications and patient care aids.
  • Ability to educate patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
  • Ability to prepare and process laboratory samples using established protocols.
  • Ability to observe, assesses, and record symptoms, reactions, and progress.
Physical Demands

Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye‑hand coordination.

Working Conditions

Work is normally performed in a typical interior/office work environment. Considerable physical activity.
