We are seeking a per‑diem Nurse Practitioner (FNP, ANP, AGNP) or Physician Assistant (PA) to provide compassionate care in Skilled Nursing Facilities (SNFs) and independent‑living communities in the Bloomfield, CT area, supporting geriatric patients.

• Perform routine physical examinations and develop medical, educational, and psychosocial care plans.
• Collaborate with physicians and care teams to evaluate health needs and refer to specialists as required.
• Provide appropriate treatment within provider scope per protocols; identify outside‑scope issues and refer to physicians.

• Licensure:
  Unrestricted State of Connecticut medical license (or eligible).
• Training: APRN (Family, Adult/Geriatric Nurse Practitioner) or PA certification.
• Experience:
  
  Minimum two years of post‑graduate experience in a skilled nursing or geriatric setting.
