Home and Community Based Services Licensed Practical Nurse

You will provide direct nursing care as prescribed by the physician to acutely and/or chronically ill patients in their homes, record clinical and progress notes, and coordinate other patient care activities to deliver the highest quality of care within your assigned geographic service area. You will work under the direct supervision of a Registered Nurse or designated supervisor.

Primary Responsibilities
  • Provide high‑quality clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other members of the health‑care team, following all applicable infection control procedures.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record and, as applicable, in the agency’s electronic visit verification program.
  • Actively participate in the agency’s care coordination process, reporting patients’ needs and changes in condition in a timely manner, attending patient case conferences, and communicating effectively with patients, families, physicians, other clinicians, agency management staff, case coordinators, and community case managers.
Required Qualifications
  • Current and unrestricted Licensed Practical Nurse licensure in the state of practice.
  • Current CPR certification.
  • Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation.
  • Ability to function in any home care setting regardless of client’s age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications
  • Home care experience.
  • Ability to work flexible hours as required to meet client needs.
  • Ability to manage multiple tasks simultaneously and work independently.
  • Good communication, writing, and organizational skills.

The hourly pay for this role will range from $20.00 to $35.72 per hour based on full‑time employment, local labor markets, education, work experience, certifications, and other factors.
