Week 1:
Tuesday, Wednesday, Saturday. Week 2:
Monday, Thursday, Friday. No on call.

Primary work schedule is 3 days a week with occasional Saturday work.

DaVita Kidney Care is bringing industry‑leading clinical quality to patients on‑site at skilled nursing facilities (SNFs) through a new team called DaVita Skilled Nursing Dialysis Services. Approximately 10% of all dialysis patients reside in an SNF and are transported to dialysis three times a week, creating a significant burden on patients and adding avoidable costs. DSD Services teammates will work to quickly leverage DaVita’s resources to develop infrastructure and grow this new service line in a start‑up environment.

Ideal teammates have experience in dialysis, are energized by a start‑up type environment, and have a track record of getting things done (GSD). They must be comfortable working autonomously and collaborate effectively with cross‑functional resources.

• Provide complete ongoing care for each patient, ensuring dialysis treatments stay on track.
• Troubleshoot patient issues, make sound clinical decisions, and partner with SNF staff to coordinate care.
• Conduct patient and family education in the SNF setting.
• Supervise Dialysis Technicians delivering tandem care to meet capacity and efficiency goals.
• Apply critical thinking and nursing skills to assess, diagnose and implement comprehensive care plans.

• Current Registered Nurse (RN) license in the state of practice.
• Minimum of 12 months of RN experience.
• Associates Degree in Nursing (ADN) from an accredited nursing school required; BSN or a three‑year diploma from an accredited diploma program may substitute the degree.
• Basic computer skills and proficiency in MS Word and Outlook.

• Previous experience in Chronic Hemodialysis, Acute Dialysis and/or Peritoneal Dialysis (PD).
• Background in training, teaching, or healthcare education.
• Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN).
